• Request for Duplicate Documents

Used to obtain copies of POST issued certificates and other documents related to officer employment and training. Please allow up to 5 business days for processing and returns.

• Affidavit

This form must be completed, notarized and attached to the Basic Application Form. Must be attached to Employee PAR ONLY for a REHIRE/LATERAL officer.

• Investigation Form

In order to assist agencies with compliance with new statute NRS 289.585, use this form and submit to Chief Kathy Floyd at kfloyd@post.state.nv.us

• PPRT Acknowledgement Form

Completed form must be submitted with the application for POST Basic Certificate. If POST does not proctor the test for you, include the PT scorecard with the application as well. Use this form for Reciprocity or the Recertification Program.

Basic Certification
